Please note that all new applicants are subject to our credentialing process, ensuring transparency and fairness in evaluating their qualifications. Credentialing form

Right of Review: As an applicant, you have the right to review non-privileged information used to evaluate your application. Submit a written request to the Credentialing Department, and they will arrange a review within 30 working days.

Notification of Discrepancy: You will be notified in writing if information from primary sources significantly differs from what you provided. Source information not intended for credential verification or protected by law will not be revealed.

Correction of Erroneous Information: If you believe there is erroneous information from primary sources, notify the Credentialing Department with a detailed explanation within 30 days. The Medical Group will re-verify the information, make necessary corrections, and notify you. Failure to respond within the given timeframe will result in forwarding the original information to the Credentialing Committee for evaluation and decision.
