Provider Demographic Update

It is very important that we have current, and up-to-date information related to your practice. Examples of reportable items include changes to:

  • Practice address

  • Practice contact information

  • Practice Tax Identification Number (TIN)

  • Remittance and payment address

  • Hours of operation

  • Panel statuses (accepting or not accepting new patients)

  • New associates joining your practice (including mid-level practitioners)

 Please report changes to our Provider Relations Department using the following forms:

Claim Forms

Payments made by HHMG to the providers will automatically be deposited into the bank account upon completion of EFT process.  Additionally, providers will automatically receive Electronic Remittance Advice (ERA) service.

Credentialing Forms

To deliver safe and high-quality healthcare services, all providers must complete primary source verification by completing and submitting the following application.

Provider Portal Access Form

HHMG offers the use of a convenient online Provider Portal to all contracted providers. Our portal will provide access to:

  • Patient Eligibility

  • Claims information (Capitation EoB)

HHMG Provider Manual

  • To obtain a copy of our Provider Manual, we kindly request that you contact our Provider Relations Department. Alternatively, you may access the manual through your personalized HHMG share folder.

All employees, contractors, providers, and volunteers are free to request a copy of HHMG policies and procedures for better understanding. Policy and procedures appendices may be downloaded here: P&Ps Appendices.
