The health care industry in the United States is one of the most complex and regulated. To ensure we have a common understanding of how our values apply to our business decisions and have an effective line of communication, we appreciate and expect all employees, Providers, Contractors, and Downstream Entities comply with Compliance Training.  The purpose of training is to Prevent, Detect, Report, and Correct non-compliant practices.  Please complete and attest to having reviewed and completed all the following trainings on an annual basis:

We require all providers, employees, and FDRs to sign an annual FDR Attestation. This attestation helps us demonstrate to CMS that we're effectively communicating their requirements, as well as confirming that our FDRs are complying with them. Use the appropriate attestation form below:

HIPAA & Privacy

Hana Hou Medical Group is committed to protecting the privacy of personal health information and abides by HIPAA (Health Insurance Portability Accountability Act) law.

Our Notice of Privacy Practices (see below) describes the different ways in which we or our affiliated contractors may use of disclose protected health information. It is imperative that all affiliated medical contractors comply with HIPAA regulations.

NOTIFICATION OF PRIVACY PRACTICES

We are committed to maintaining and protecting the confidentiality of our members’ medical and financial information which we refer to as PHI (Protected Health Information).    We are required by law to maintain the privacy of your health information and to provide you with this notice. We must follow the terms of this Notice while it is in effect.

Protected health information (PHI) is any information in the medical record or designated record set that can be used to identify an individual and that was created, used, or disclosed while providing a health care service such as diagnosis or treatment.  Protected Health Information includes the person's participation in the program, the person's past, present or future physical or mental health condition, the provision of health care to that person, and payment for the provision of health care to that person. Protected Health Information does not include publicly available information or information that is available or reported in a summarized or aggregate fashion that does not identify any individual person.

This notice explains how we use information about you and when we can share that information with others. It also informs you of your rights with respect to your health information and how you can exercise those rights.

The following are ways we may use or share information about you:

  • May disclose personal information about members that physicians or other health care providers request in order to help with medical treatment or services. For example, if you are in the hospital, we may give them access to any medical records sent to us by your doctor.

  • May use the information to help pay your medical bills that have been submitted to us by doctors and hospitals for payment.

  • May use and disclose personal information for health plan operations.

  • May use or disclose personal information when required by federal state or local law. For example, we may disclose personal information to a health oversight agency, to include the Secretary of the Department of Health and Human Services or a state insurance department, for activities such as audits, investigations or related to licensure.

  • May disclose personal information to law enforcement officials if we receive a court order, warrant, grand jury subpoena or inquiry for purposes of identifying or locating a suspect, fugitive, material witness or missing person.

  • May report information to a government authority regarding child abuse, neglect or domestic violence.

  • May share information with a coroner or medical examiner to identify a deceased person, determine a cause of death, or as authorized by law. We may also share information to a funeral director as necessary to carry out their duties.

  • May use or share information for procurement, banking or transplantation of organs, eyes, or tissue.

You have the right to receive a copy of this notice upon request at any time. You can also view a copy of the notice on our web site. Should any of our privacy practices change, we reserve the right to change the terms of this notice.  We will make the new notice effective for all protected health information we maintain. Once revised, we will provide the new notice to you upon request and post it on our website.

  • Medicaid (QUEST) – Plan members should address their concerns to the Med-QUEST Customer Service Section at 524-3370 (Oahu) and (800)316-8005 (neighbor island) and health care providers should present their concerns to the Med-QUEST Provider Hotline at 692-8099.

WE WILL NOT TAKE ANY ACTION AGAINST YOU FOR FILING A COMPLAINT.

We reserve the right to change our practices and to make the amended provisions effective for all individual identifiable health information that we maintain. If we change our privacy practices, we will post a revised notice within 60 days of the revision and provide a copy to you upon your request.
